Trainees can send out requests for work-place based assessments to assessors from their FOM Portfolio Builder accounts. Assessors will receive an email notifying them of the assessment request. Assessors with FOM Portfolio Builder accounts will also be able to access the assessments from the dashboard of their own account.
Step 1: Click on the '+ New Assessment' button on your Specialty Training Portfolio card on the dashboard of your FOM Portfolio Builder account.
Step 2: Select the standard you wish to be assessed against and the assessment type.
Step 3: Click on 'Create Assessment'.
Step 4: Complete the details on the form.
The greyed-out sections of a form can only be completed by the nominated assessor.
Step 5: Click on 'Submit'.
If you wish to save the assessment as a draft and submit it to the assessor for completion at a later date, click on 'Save Changes'. The information you enter remains private until it is submitted. Once the form has been submitted you cannot make any more changes.
Step 6: Enter the email address of the assessor you wish to complete the assessment.
The name of the assessor will be entered automatically if the email address is registered to a FOM Portfolio Builder account. If not, enter the name of the assessor manually.
Step 7: Click on 'Nominate'.
Step 8: Click on 'Send'.
Assessment requests expire after 28 days and after this time can be deleted by the trainee if they are no longer required.
